Job Summary
St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager for the Women's Night Program. The case manager will provide counseling and housing-focused case management with clients and establish relationships built on trust and collaboration with activities consistent with the clients' housing goals, among other duties. Moreover, the Case Manager will need to have experience working with individuals experiencing mental health and substance use disorders, as well as case management experience. This position will require availability to work Monday through Friday, 11am to 7:30pm. The salary range is between $40,000-$50,000.
Job Responsibilities
Work closely with Women’s Night Program Manage
Conduct intake on shelter referrals
Manage caseload of up to 15-17 clients
Provide housing focused case management for WNP clients.
Crisis intervention with clients
Conduct groups with clients
Maintain accurate filing system.
Input information into CaseWorthy, HMIS and update regularly
Efficient in HMIS and keep records updated.
Provide weekly updates on clients.
Collaborate with partner agencies to address needs of clients.
Provide initial and ongoing assessment of clients’ progress towards housing opportunities.
Assist clients with implementing short- and long-term housing and financial stability goals using strengths and preferences as foundation to build goals.
Develop housing stability plan with all clients with input from other treatment providers, family members and community supports, as applicable and update plan bi-weekly at minimum
Assist clients with financial stability, including but not limited to obtaining employment, and/or government benefits.
Accept shelter referrals on weekends.
Maintain integrity of shelter on weekends
Rotate on-call weeks with other case manager.
Foster strong working relationships and provide consistent communication with communitybased organizations and partner agencies to ensure coordination of services.
Maintain database of current and community partners
Participate in team meetings and communicate individually with team members.
Coordinate services and generate ideas to help clients achieve housing goals.
Transport clients to community partner appointments
Other duties as assigned.

Pre-Employment Screening
