Aftercare Case Manager

Job Responsibilities

Responsible for 6 to 8 aftercare cases, to include:

  • Complete and document weekly home visits for each resident in the first month of aftercare followed by every other week for the next five months (Marygrove vehicle should be used when possible)
  • Assist family in locating needed resources
  • Conduct treatment team meetings at 60 and 150 days post residential discharge
  • Complete the monthly aftercare plan
  • Provide 24-hour on-call crisis intervention and support to family (via phone or virtual)
  • Attend any FSTs, staffing, etc. that occur prior to transition to aftercare
  • Complete discharge summary for resident when they leave residential/start aftercare and at the conclusion of aftercare

Assist with residential case manager duties when needed/when not at full case load, including:            

  • Supervising calls and visits           
  • Transporting to court and psychological appointments
  • Completing psychosocial summaries, initial CANS assessment, and discharge summaries
  • Attend weekly clinical meeting, monthly trauma-informed supervision, and any other required agency meetings/trainings
  • Other duties as delegated by supervisor

Job Requirements

  • Bachelor’s degree in social work, psychology, or related area of study from an accredited school
  • Experience working with children and families in a residential-type setting preferred
  • Ability to work as a team and independently

Pre-Employment Screening
