Inspired by the teachings of Jesus Christ, the mission of Cardinal Ritter Senior Services is to be a recognized provider in giving person centered care through an integrated continuum of quality residential, healthcare, and supportive social services for senior adults throughout the Archdiocese of Saint Louis.
A Service Coordinator (social services professional) is a staff member available to all residents living in Cardinal Ritter Senior Services’ affordable senior apartments. A Service Coordinator is responsible for assuring that residents, especially those who are frail or at-risk, are linked to the supportive services they might need to continue living independently and safely in their apartments, thus preventing premature and inappropriate relocation to long term care facilities.
DUTIES & RESPONSIBILITIES
- Assess the needs of the apartments’ residents, with focus on their activities of daily living, monitoring them quarterly and/or re-assessing them yearly, depending on their priority (frail, at risk, or non-applicable).
- Refer and link residents to service providers in the community to provide services that include, but are not limited to, homemaker services, personal care services, meal/food programs, transportation services, money management services, legal services, mental health services, State/Federal benefit programs, Medicare/Medicaid and other health insurance information, long term care information, etc.
- Monitor the delivery of services to residents to ensure they are appropriate, timely, and satisfactory.
- Establish links with community agencies and service providers, creating resource information/lists that are available to residents and families.
- Educate residents on available community resources, service availability, application procedures, client rights, etc., both individually and as a group.
- Assist residents with applying/re-applying for state and federal benefit programs.
- Advocate for residents when they have reached an impasse or barrier to services, or are not able to advocate for themselves.
- Encourage residents to be proactive in meeting their social, psychological, and physical needs.
- Educate apartment staff on issues related to aging in place and coordination of services, to help them better work with and assist the residents.
- Assist apartment management in identifying residents who need assistance, as well as following up on referrals from management related to specific residents and their need for supportive services by using the least restrictive intervention necessary to alleviate a problem situation.
- Report all suspected abuse, neglect, and exploitation situations to the appropriate agency.
- Document all contacts with residents, providers, families, etc.
- Maintain secure individual resident files in a confidential manner, which will contain at least the following: release of information forms, casenotes, assessment forms, monitors/reassessments, agency provider information, copies of applications/letters related to the resident, etc.
- Complete all reports/statistics required by the supervisor, agency, and HUD in an accurate and timely manner.
- Attend all training required by HUD within the first year of employment as a Service Coordinator and each year following.
- Perform other duties as assigned.
KNOWLEDGE & EXPERIENCE REQUIREMENTS
- Baccalaureate degree required - concentration in Social Work or Gerontology ideal.
- Two years experience with provision of services to senior adults desired with a demonstrated working knowledge of community services/providers available to this population.
- Awareness of eligibility for and procedures of State and Federal benefit programs preferred.
- Training in subjects that include, but are not limited to, the aging process, senior adult services, disability services, drug and alcohol abuse, mental health issues, etc. helpful.

- As employees of a Catholic agency which cares for senior adults, all staff will demonstrate a respect for each client as a person created in the image of God, in a non-ageist manner.
- Good communication, writing, problem solving, and organizational skills.
- Strong advocacy capabilities, while respecting the resident’s right to self-determination.
- Ability to follow-up with requests for assistance made by residents, families, apartment management staff, and agency administration within a timely manner and provide an appropriate response, within confidentiality guidelines, as quickly as possible.
- Ability to work in a flexible manner and relate well within a team approach necessary.
- Ability to meet deadlines related to position paperwork a must.
- Willing and able to take direction from a supervisor.
- Proficient working knowledge of Microsoft Office, e-mail and the internet.
- Must be able to express the four CRSS Mission Alive components in work tasks, i.e., love, hope, compassion and integrity.
RELATIONSHIPS REQUIREMENTS

- Will frequently deal with people of varying ages and must be able to cope with requests by residents or family members in an appropriate manner.
- Will interact with interdisciplinary team members within the agency on a regular basis.
- Must work with a wide variety of community organizations and governmental agencies.
RESOURCES FOR WHICH ACCOUNTABLE
- The resident population will include senior adults, 62 and over, and some adults under the age of 62, who need an accessible apartment due to hearing, vision, or mobility disability. An individual Service Coordinator may serve up to 3 affordable senior apartment sites and up to 210 units.

Pre-Employment Screening
All candidates receiving an employment offer must submit a pre-employment screening. The screenings will include criminal background check and other background checks (as needed), Family Care Safety Registration, drug screen, an employment and education or licensure/certification verification, and Covid-19 and Influenza vaccination. Various positions will require physical examination, Tuberculosis screening and Hepatitis A. All offers are contingent upon successful completion of required screening.
