Job Summary

St. Patrick Center, a ministry of Catholic Charities is hiring a coordinator for the Shallow Subsity program. Engaging with client identity community needs is a large aspect of this role. As part of the Shallow Subsidy team, the coordinator will ensure that individualized housing plans best suit the client's needs are established in a timely manner. The case manager will use best practices to assess the housing needs of veterans who are at risk of becoming or are currently unhoused and utilize a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability. The core working hours are Monday through Friday 8:00 am to 4:30 pm. The starting salary for this position is between $40,000 and $50,000 a year. 

Job Responsibilities

  • Assures monthly case management interviews are held to support individualized housing plan of client.                                                                                                                                                                                                                 
  • Assures monthly rents are paid in a timely manner.
  • When needed assist with housing placement based on the desires of clients.
  • Work with the case management team to monitor clients progress in obtaining housing goals.
  • Enlist the support of HCN’s regarding therapeutic interventions.
  • Make appropriate referrals either to SPC Programs or SPC Agency Partners
  • Ensure clients are coached in problem solving and long-range planning.
  • Assures appropriate referrals are made to SPC Programs and or SPC Agency Partners
  • Assures completion and goal attainment of indicators as outlined in the SS grant.
  • Assess the client’s strengths, desires, and immediate/long term needs.
  • Collaborate with family and relevant referral supports which exist in the community.
  • Act with immediacy pertaining to input from case managers regarding employment, housing, mentoring, substance abuse, and other immediate/long term needs.
  • Complete daily and monthly departmental paperwork
  • Assures all client services are entered into CaseWorthy correctly.
  • Assures adherence to the strategies and goals as outlined in the client’s housing plan.
  • Assures team members are addressing needs of client by providing needed services identified on individualized housing plan.
  • Assures case managers research additional benefits to assist the client in improving their situation and appointments are made whenever possible.
  • Other duties as assigned.

Job Requirements

  • Ability to perform and supervise effective case management services.
  • Ability to comply with grant requirements, including submitting required grant reports.
  • Knowledge of housing and other supportive service resources
  • Ability to engage and work with community agencies and programs.
  • Interest in helping veterans reach their full potential.
  • Ability to work well with a wide range of people in various settings.
  • Excellent computer skills and the ability to process large amounts of data.
  • Ability to handle confidential information.
  • Ability to remain calm in stressful situations.
  • Ability to manage a caseload and supervise reports at the same time.
  • ECM computer entry and use of Excel spreadsheets
  • Knowledge of various programs throughout the SPC Partnership Center
  • Ability to obtain vouchers for transportation, clothing, food, furniture, and other needs.

Skills Required for Success in Job

  • College degree or 4 to 6 years equivalent professional experience required.
  • Driver License required (MO residents requires Class E license)
  • Valid and current auto insurance and reliable vehicle
  • Ability to work a minimum of 37.5 hours per week.

Pre-Employment Screening
