Job Summary

The Case Worker at Sts. Joachim and Ann Care Service provides compassionate, client-centered support to individuals and families in need. This role focuses on assessing client needs, developing and coordinating service plans, advocating for resources, and promoting stability, dignity, and overall well-being. The Case Worker works collaboratively with internal teams, community partners, and service providers while upholding the mission and values of Sts. Joachim and Ann Care Service.

Job Responsibilities

  • Conduct comprehensive client intakes, assessments, and ongoing evaluations

  • Develop, implement, and monitor individualized service or care plans

  • Provide case management, referrals, and follow-up services

  • Advocate for clients to access housing, healthcare, financial assistance, education, and other community resources

  • Maintain accurate, timely, and confidential client records and documentation

  • Coordinate services with internal staff, external agencies, and community partners

  • Support clients in goal-setting and progress toward self-sufficiency

  • Monitor client progress and adjust service plans as needed

  • Participate in team meetings, case conferences, and required trainings

  • Ensure compliance with agency policies, ethical standards, and applicable regulations

  • Demonstrate cultural sensitivity, professionalism, and compassion in all interactions

Job Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (required or preferred, as applicable)

  • Previous experience in case management, social services, or a related setting preferred

  • Experience working with vulnerable populations is a plus

  • Strong interpersonal, communication, and advocacy skills

  • Ability to build trusting relationships with clients from diverse backgrounds

  • Excellent organizational and documentation skills

  • Knowledge of community resources and social service systems

  • Ability to work independently and as part of a multidisciplinary team

  • Strong problem-solving and time-management abilities
