Job Summary

St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager for Project LIVE, to provide client support in our Permanent Supportive Housing Program. The Case Manager will offer housing-based case management support and advocacy to program participants who are chronically homeless, among other duties. As a part of the Project LIVE team, this case manager is responsible for creating treatment plans, collaborating with program managers, and providing stability to clients. Moreover, the Case Manager will need to have experience working with the unhoused population and conducting home visits. The core working hours are Monday through Friday from 8 am to 4:30 pm, and the salary range is between $40,000 and $50,000.

Job Responsibilities

  • Assist program participants with identifying housing.
  • Conduct home visit meetings
  • Perform crisis interventions.
  • Practice the Housing Based Case Management training model.
  • Conduct Housing Based Case Management assessment meetings.
  • Develop and update treatment plans with an emphasis on fostering personal growth and development towards housing stability, mental health, and physical wellness.
  • Assist program participants with activities of daily Living .
  • Provide transportation assistance.
  • Assist program participants with applying for SSI/SSDI benefits.
  • Provide internal and external referrals.
  • Conduct Housing Quality Standards inspections
  • Collaborate with in-patient/out-patient treatment programs and other service providers.
  • Participate in weekly crisis team meetings.
  • Attend staff meetings.
  • Attend annual program staff training.
  • Update client services and client progress in Case Worthy system
  • Update Service Point HMIS and Case Worthy systems for data quality standards
  • Other duties as assigned.

Job Requirements

  • High school diploma or GED required.
  • Excellent organizational and planning skills
  • Excellent critical thinking skills
  • Experience working with chronic homeless population.
  • Experienced in conducting home visit meetings.
  • Skilled in de-escalation techniques
  • Knowledge of Housing First Model
  • Proficient in Microsoft Office programs
  • High level of sound judgement and discretion
  • Ability to multi-task
  • Ability to work in team atmosphere.

 

SKILLS REQUIRED FOR SUCCESS IN JOB

  • Trained in Motivational Interviewing
  • Driver’s license required (MO residents require a Class E)
  • Reliable vehicle and current auto insurance required.

 

PREFERRED QUALIFICATIONS

  • Bachelor’s degree in social work or related field preferred
