St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. 

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. 

Position Summay

St. Patrick Center, a ministry of Catholic Charities, is seeking a compassionate and dedicated case manager to join the Hospital to Housing team. As the case manager, you will work collaboratively with hospitals and agencies to ensure that services and essential resources are provided in a timely manner. The case manager plays a pivotal role in supporting unhoused clients to engage with the community. While partnering with the Hospital to Housing team, you will assist individuals exiting homelessness and entering housing by facilitating the move-in process, coordinating appointments, and establishing long-term financial habits that support long-term goals. The core working hours of the St. Patrick Center are Monday through Friday, 8:00 am to 4:30 pm. 

Overview and Responsibilities

•    Work directly with the hospital social workers to engage, accept and process referrals to the program.

•    Conduct comprehensive, ongoing assessments of clients’ mental health, physical health, housing status, other basic needs, and barriers that impact optimal wellbeing in the community.

•    Facilitate the move-in process which includes the housing application, rent payment, move-in kits and actual move-in within 30 days of referral.

•    Provide on-call crisis intervention in coordination with other H2HH team members, covering nighttime hours, holidays, and weekends.

•    Manage primary case load.

•    Facilitate person centered, strengths base case management services to individual clients in the office, hospital and in community settings.

•    Refer and assist in warm handoffs to appropriate community agencies, consistent with evolving service planning.

•    Coordinate and help clients with making and keeping appointments and any other tasks within medically necessity of client’s recovery process.

•    Act as advocate and liaison, consulting with community agencies and providers to maintain coordination of care and transition.

•    Collaborate with other members of the H2HH team during daily and weekly staff meetings.

•    Maintain St. Patrick Center client tracking system and client files (i.e., assessment, service plans, etc.), documenting client contact and progress; to maintain a permanent record of client activity within the timeframe required (24 hours)

•    Responsible for agency policy regarding work hours, building security, and all written personnel standards.

•    Perform within St Patrick Center’s adopted care models: Housing First, Trauma Informed Services and Recovery Oriented

•    Other duties as assigned.

Knowledge and Experience Requirements

•    Bachelor’s degree in social work, counseling or a related field from an accredited university and/or associate degree with 2 to 4 years’ experience accepted.

•    Peer Support Certification preferred.

•    Requires driver’s license (MO residents require Class E)

•    Requires reliable vehicle and valid and current auto insurance.

•    Knowledge and understanding of community agencies.

•    Knowledge and understanding of Housing First Model

•    Knowledge and understanding of Trauma Informed Care

•    Knowledge of federal, state, and agency HIPAA policies

Skills and Competencies Required

•    Understanding of mental illness and substance use

•    Understanding of strengths base case management/wrap-around services

•    Excellent verbal and written communication skills

•    Crisis intervention expertise

•    Self-motivation due to the autonomous nature of the position

•    Respect for clients and staff

•    Sensitivity to cultural diversity

•    Ability to work in a team atmosphere.

•    Ability to work in high pace organization.

•    Moderate computer skills

•    Work a minimum of 37.5 hours a week.
