St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. 

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. 

Position Summay

St. Patrick Center, a ministry of Catholic Charities, is hiring a case manager to ensure the safety and well-being of residents by observing and evaluating daily activities within the program. This position is responsible for monitoring referrals, conducting intakes, and providing support during crisis events. The Case Manager will collaborate with team members to address resident needs effectively and implement appropriate interventions. Additionally, the Case Manager will participate in an on-call schedule, providing assistance during after-hours crises to ensure continuous support and management of any emergencies. The core working hours of the St. Patrick Center are Monday through Friday, 8:00 am to 4:30 pm.

Overview and Responsibilities

•    Direct individuals to appropriate entrances such as volunteers, visitors and persons seeking services.

•    Work varying shifts and participate in an on-call schedule.

•    Receive and accept incoming referrals via the Get Help app.

•    Complete intakes, VI-SPDATS, wellness screenings, and referral navigation when residents arrive.

•    Work with Safe Haven Director to develop Community Calendar of engagement activities.

•    As issues arise, provide de-escalation, conflict resolution, crisis response, and removal of guests if necessary.

•    Complete security rounds both inside and outside the site to monitor the safety and cleanliness of the area.

•    Engage clients in conversation to determine needs and provide appropriate resources.

•    Inform individuals in need of food, shelter, medical, and hygiene assistance of the location of resources.

•    Engage clients in conversation regarding substance use, mental health issues and other issues that keep them on the street.

•    Work closely with area service agencies to ensure service delivery.

•    Refer clients to appropriate community agencies and shelter providers as needed.

•    Use appropriate de-escalation techniques and judgement to call “911” for emergency assistance.

•    Provide a welcome presence to persons seeking services

•    Maintain notes and record services provided for clients in Case Worthy and Service Point

•    Complete daily, weekly, and monthly reports

•    Complete agency Incident Reports as needed.

•    Transport clients to community partner appointments

•    Maintain clean and professional appearance.

•    Develop relationships with community outreach workers, city/state officials, and St. Patrick Center staff.

•    Communicate verbally and in writing with the Director of Safe Haven

•    Other duties as assigned.

Knowledge and Experience Requirements

•    Bachelor’s Degree and/or Peer Support Certificate required or equivalent of 4 to 6 years of professional experience will be considered.

•    Requires driver license (MO residents require Class E)

•    Requires valid and current auto insurance.

•    Requires reliable vehicle.

•    Work a minimum of 37.5 hours a week.

Pre-Employment Screening
