St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability.
Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.
Position Summary
St. Patrick Center, a ministry of Catholic Charities, is hiring a Prevention Case Manager to play a vital role in helping individuals and families at risk of eviction and homelessness find stability and a place to call home. In the position, you will utilize grant resources, work hand-in-hand with clients to uncover their personal support networks, and connect them with a range of wraparound services offered at St. Patrick Center. Through collaboration with clients and community partners, you will empower them to achieve permanent housing solutions. The core working hours are Monday through Friday, 8:00 am to 4:30 pm.
Overview and Responsibilites
• Complete program intakes/assessments on all households
• Maintain up to date client files.
• Develop service/needs assessment plans to identify and address barriers to maintaining housing.
• Connect households with prevention funding that best fits each family’s needs.
• Assist households in creating budgets.
• Help households increase income through employment and/or benefits.
• Assist clients in accessing assistance offered by community programs including assistance with security deposits, moving expenses, etc.
• Establish and maintain excellent working relationships with area housing resource providers, landlords, and related services/agencies.
• Ensure all paperwork is completely properly by client and uploaded into HMIS.
• Track and report outcomes to Program Manager
• Other duties as assigned.
Knowledge and Experience Requirements
• High school diploma or GED required.
• Degree in Human Services, Counseling, Social Work, or related field preferred
• Experience working with socially/economically disadvantaged persons preferred.
• Driver license required (MO residents require a Class E)
• Valid and current auto insurance required.
• Reliable vehicle required.
• Knowledge of Microsoft Office 365 and data entry

Pre-Employment Screening
