St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. 

Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. 

Position Summary

St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager to join our Housing, Employment, and Recovery Opportunities (HERO) program. In this rewarding role, you will engage with clients residing in transitional housing, offering guidance and support tailored to their unique needs. You'll work collaboratively with clients to strengthen their connections with family and community resources while actively monitoring their progress on individualized plans. By coordinating personalized services and fostering relationships with service providers both at home and in the community, you will be instrumental in facilitating their journey towards stability and independence. The core working hours are Monday through Friday, 8:00 am to 4:30 pm.

Overview and Reponsiibilites

•    Visit clients weekly to track ISS goals.

•    Transport clients to community partner appointments

•    Contribute to the ideas and processes of the case management team.

•    Use a peer-to-peer approach to build rapport with individuals in recovery.

•    Provide emotional and informational support for permanent housing by assessing strengths, desires and immediate and long term needs of each client.

•    Develop or contact family and natural supports which exist in the community.

•    Receive input from case management team members regarding employment, housing, mentoring, substance use, other immediate/long term needs, and long-term financial stability needs.

•    Complete daily and monthly departmental paperwork

•    Daily update of electronic files including contact with clients, services or any internal or external referrals made.

•    Adhere to the strategies and goals outlined in the ISS and Treatment Plan

•    Coach clients to make solid well-informed decisions which maintain housing and advocate for the client.

•    Assist client with monthly budgeting.

•    Complete treatment plans and track goals monthly

•    Provide financial and money management counseling sessions.

•    Provide weekly case management sessions to ensure service delivery.

•    Teach Life Skills classes.

•    Refer client to additional resources to address other service needs (i.e., legal assistance, VA benefits, community substance use counseling etc.)

•    Research additional benefits available to enhance service being provided.

•    Engage the client, the family and natural supports within the community setting when feasible and desired by client.

•    Networks with federal, state, and local agencies

•    Any other duties as assigned.

Knowledge and Experince Requirements

•    Bachelor’s degree required.

•    Peer Support Certification or MSW preferred.

•    Knowledge of housing and other supportive service resources

•    Knowledge of Housing First and Harm Reduction

•    Requires driver license (MO residents require Class E)

•    Requires reliable vehicle.

•    Requires valid and current auto insurance.

•    Work a minimum of 37.5 hours a week.

Pre-Employment Screening
