POSITION SUMMARY

Community Support consists of specific activities with or on behalf of a particular client in accordance with an individual rehabilitation plan to maximize the client’s adjustment and functioning within the community while achieving sobriety and sustaining recovery, maximizing the involvement of natural support systems, and promoting client independence and responsibility.

OVERVIEW AND RESPONSIBILITIES

  • Develop recovery goals and identify needs, strengths, skills, resources and supports and teaching how to use these supports, identify barriers to recovery
  • Assist in the development of treatment plans
  • Assist clients in develop natural supports to achieve and maintain recovery
  • Help clients restore skills and resources that were negatively impacted by substance use
  • Participate in the interdisciplinary team meeting in order to identify strengths and needs related to development of the individual’s rehabilitation plan including but not limited to:

a. Employment;

b. Education;

c. Housing;

d. Nutrition, meal preparation, meal planning and shopping

e. Lifestyle changes and healthy lifestyle education
and actively work with clients to complete treatment goals and objectives

  • Attend periodic meetings with designated team members and the client, whenever feasible, in order to review and update the rehabilitation plan
  • Contact clients who have no showed within 72 hours in order to re-engage the person and promote recovery efforts
  • Arrange and refer for services and resources and, when necessary, advocating obtaining the services and quality of services to which the person is entitled
  • Monitor service delivery by providers external to the program and ensuring communication and coordination of services
  • Locate and coordinate services and resources to resolve a crisis
  • Provide experiential training in life skills and resource acquisition
  • Provide information and education to an individual in accordance with the person’s rehabilitation plan
  • Plan for discharge
  • Consistently meet monthly billable expectations
  • Transport clients in employee’s vehicle
  • Provide group education
  • Assist client in establishing treatment goals
  • Schedule includes one weeknight per week and one weekend coverage per month
  • Other duties as assigned

KNOWLEDGE AND EXPERIENCE REQUIREMENTS

  • Bachelor’s degree in social work, counseling, psychology, or closely related field; or

a. A combination of higher education and qualifying experience equaling four (4) year; or

b. Associate of Applied Science in Behavioral Health Support degree from an approved institution; or

  • Four (4) years of qualifying experience
  • Valid Class E (or other acceptable) driver’s license, vehicle, and current automobile liability insurance

 

SKILLS AND COMPETENCIES REQUIRED

  • Ability to lift 20 pounds
  • Operates from person-centered, person-driven, recovery oriented, and stage-wise service delivery approach that promotes health and wellness.
  • Developed cultural competence with the ability to understand,
  • communicate with and effectively interact with people across cultures
  • Delivers services according to key service functions that are evidence-based and best practice
  • Practices in a manner that demonstrates respect and understanding of the unique needs of the client.
  • Uses effective strategies for engagement/re-engagement, relationship building and communication
  • Demonstrates an understanding of mandated reporting requirements for abuse and neglect of children and abuse, neglect or financial exploitation of senior citizens or disabled individuals
  • Shadowed a peer or supervisor in these functions of the job at least three times
  • Attends regular clinical supervision
  • Completed ASAM training
  • Demonstrates and understanding of co-occurring disorders
  • Demonstrates an understanding of suicide prevention
  • Demonstrates an understanding trauma-informed care
  • Complete CSS 101
  • Demonstrates an understanding of ethical behavior
  • Demonstrates an understanding of HIPAA
  • Demonstrates an understanding of addiction
  • Demonstrates a basic understanding of substance use disorders
  • Demonstrates a basic understanding of medications used to
  • treat substance use disorders
  • Demonstrates the signs and symptoms of acute psychiatric crises
 

Job Type: Full-time

Pay: From $37,000.00 per year
