Job Summary

Functions as a Licensed Practical Nurse (LPN) for Mary, Queen and Mother Center (MQMC).  The LPN is responsible for supervising assigned staff and providing quality care to the residents in keeping with CRSS mission and philosophy. 

Job Responsibilities

  • Uses the Nursing process of assessment, planning, intervention, and evaluation to respond to resident needs holistically.
  • Supervises, instructs, and assists Certified Medication Technicians (CMTs) and Certified Nurse Assistants (CNAs) in resident care, ensuring proper care and maintaining current assignment sheets.
  • Promotes individualized, maximum care for each resident physically, emotionally, and spiritually.
  • Supervises/administers prescribed medications and treatments per physician orders, MQMC policies, and the Nurse Practice Act.
  • Observes and records residents' reactions to medications and treatments, reporting significant incidents to the Nursing Supervisor, physician, and responsible party; documents in the resident’s chart and the 24 Hour Report.
  • Provides a complete, accurate report about residents to the next shift.
  • Assists with the Minimum Data Set (MDS) timely, using information for care plans and attending resident care conferences as scheduled.
  • Maintains medical records with all pertinent information including condition changes, summaries, Medicare notations, physician visits, skin conditions, and cognitive status.
  • Makes rounds with physicians, noting changes in resident status and completing required documentation and physician orders.
  • Completes all necessary duties for resident care, such as labs, x-rays, pharmacy requisitions, and consulting with other departments or professionals.
  • Assists with resident meals, ensuring proper feeding.
  • Completes Incident Reports for residents, staff, or visitors and notifies physician and responsible party promptly.
  • Notifies the responsible party of changes in resident condition, Incident Reports, adverse reactions, lab or x-ray permissions, consults, transfers, discharges, order changes, or death.
  • Secures and manages supplies and equipment care, use, storage, and billing.
  • Assists in orienting and teaching new staff.
  • Provides performance counseling and evaluations in collaboration with the Nursing Supervisor per policies.
  • Maintains professional growth by participating in inservices, seminars, workshops, and professional organizations.
  • Ensures communication between shifts via verbal, written, or taped reports, and informs the Nursing Supervisor of unusual incidents or changes in condition involving residents, families, visitors, or staff.
  • Performs duties professionally, showing kindness and cooperation to residents, families, visitors, and staff.
  • Starts IVs (IV Certified) and may monitor IV fluids and site once started by an IV Certified Nurse.
  • Maintains the Nursing Communication log.
  • Completes new admissions/transfers timely, verifying physician orders, faxing prescriptions, notifying departments, and documenting appropriately.
  • Destroys unused medications per procedure.
  • Ensures staff compliance with HR Policies and Procedures.
  • Performs other duties as assigned.

Job Requirements

  • Must be currently licensed and qualified as a practical nurse in the State of Missouri.
  • Knowledge in the proper use of equipment necessary to fulfill physician’s orders and facility policies.
  • Must possess a specialized interest and experience in geriatrics and Nursing care of the chronically ill and convalescent adults.
  • Must possess a knowledge or regulations governing extended care facilities.

  • Must be flexible and tolerant in circumstances as they arise.
  • Must be able to assume responsibility, supervise, and delegate duties in a fair and organized manner.
  • Must be organized, tactful, courteous, and possess good communication skills.
  • Must maintain confidentiality with all pertinent resident care information to assure residents rights are protected.
  • Must be willing to advocate resident rights to ensure fair and equitable treatment, self determinations, individuality, privacy, property and civil rights, and the right to wage a grievance.
  • Must set an example for staff in regards to promptness, manner, professionalism, dress/appearance, demeanor, safety, cooperation, customer service, Nursing Service, etc.
  • Must be able to perform the physical/mental functions/requirements of the position (see attached).

Pre-Employment Screening
