Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. 

Job Summary

St. Patrick Center, a ministry of Catholic Charities of St. Louis, is hiring a Registered Nurse to join the team at Rosati Place. The registered nurse will administer medications, maintain medication log, complete assessments, oversee medication adherence, administer injections, assist case managers with medical needs, and work with other professional staff on medical services provided to clients. St. Patrick Center's core working hours are Monday through Friday, 8am to 4:30pm.

Job Responsibilities

•    Complete initial physical assessments upon program entry 
•    Update physical assessments annually
•    Manage pill planners, update Medication Administration Records (MAR), manage all refills, and administer injections
•    Assist in health referrals and scheduling doctor appointments 
•    Assist clients in understanding the importance of medication adherence and their diagnoses
•    Conduct on going assessments of health needs and check-in daily to assess for any health updates or issues of clients
•    Accessible and available for client questions and unplanned illness/injury 
•    Facilitate health related classes i.e. nutrition, diabetes management, medication recognition/importance 
•    Facilitate room checks and health needs of clients
•    Maintain communication with supervising RN or physician
•    Screen individuals that are ill and refer to physician if needed
•    Order, transcribe, review, monitor, and discontinue medication as ordered by physician and dispose of unused medication per policy and report adverse reactions and appropriate follow-up to phone orders to assure signed order is recorded
•    Ensure monthly MARs are recorded and free of errors at the time of review for each client
•    Accurately document nursing actions and nursing summaries in timely manner
•    Ensure that infection control policies and practice for prevention of transmission of disease are evidenced by the use of appropriate hand washing, glove usage, and surface disinfection
•    Arrange for and ensure completion of medical appointments and follow-up as indicated
•    Communicate health issues and recommendations to staff, other health providers, and families
•    Coordinate hospital admissions, discharges, and follow-ups
•    Establish and maintain professional rapport with clients, families, and other health providers
•    Ensure the completion of progress notes summarizing the current monthly health status of clients to include physician visits, laboratory results, diagnostic testing, dental hygiene, and changes in health status
•    Ensure that furnishings, equipment, and supplies are maintained within guidelines established to prevent disease 
•    Work closely with Rosati staff, managers and partner with clients, physicians, and psychiatrists
•    Meet all requirements of working in a Residential Care Facility 
•    Other duties as assigned 

Job Requirements

•    Registered Nurse required
•    Drug test to be completed within 30 days of hire
•    Requires a Driver License (MO residents require Class E)
•    Requires reliable transportation and valid and current auto insurance
•    Knowledge of counseling and crisis intervention skills
•    Knowledge and understanding of area and community resources to provide appropriate referrals
•    Knowledge of Federal, State, and Agency policies of HIPAA confidentiality
•    Experience working independently and in team-oriented environment is essential
•    Experience in counseling and case management

•    Understanding of mental illness and substance use
•    Understanding of the Housing First model
•    Ability to work with challenging clients
•    High level of confidentiality, sound judgment, and discretion is required
•    Excellent written and verbal communication and computer skills
•    Ability to be detail oriented, accurate, and reliable
•    Ability to conduct the role ethically and follow all compliance requirements
•    Self-motivated with the ability to multi-task in fast-paced environment
•    Respectful to peers and clients and sensitive to cultural diversity 
•    Work a minimum of 37.5 hours a week
