Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.
Position Summay
St. Patrick Center, a ministry of Catholic Charities, is hiring a Case Manager to collaborate with St. Patrick Center and various Continuum of Care (CoC) agencies to successfully establish housing for clients and provide them with the education and skills needed to maintain self-sufficiency. Your responsibilities will include developing service assessment plans to identify barriers to housing, assisting clients in gathering necessary documentation, and partnering with outreach teams to obtain homeless verifications. As a key member of our team, you will actively connect clients to community programs that can provide financial support for moving expenses and security deposits. You'll work closely with Housing Navigators to discover housing opportunities and aid clients in securing income through employment or social security benefits. Your efforts will foster strong relationships with local housing resource providers, ensuring clients receive comprehensive support. With a focus on follow-up and documentation, you will track progress and make follow-up calls to measure success and address ongoing needs. Overview and Responsibilities
• Work with St. Patrick Center (SPC) and other CoC agencies to ensure success of establishing housing for client with housing referral
• Provide education and skills to client to maintain self-sufficiency by partnering with OT and OT students
• Develop service/needs assessment plans to identify and address barriers to finding housing
• Assist client by gathering appropriate documentation for housing while referral is in process
• Partner with Street Outreach to obtain homeless verification for client not connected with another agency
• Collaborate with CE Housing Navigator to identify new and existing housing opportunities
• Assist client in accessing community programs including security deposits, moving expenses, etc.
• Make appropriate referrals to aid client in securing income such as employment or social security benefits
• Assist with referrals to agencies that can offer employment assistance to client able to work
• Provide transportation assistance to assist individuals and families during housing search process
• Establish and maintain excellent working relationships with area housing resource providers and related services/agencies
• Create an initial budget plan at intake with client who have income or who secure income prior to housing program enrollment
• Update HMIS for all individuals and families at-risk of or experiencing homelessness
• Ensure all paperwork is filled out properly by client and uploaded into HMIS
• Make 60 Day Follow-up calls to clients referred to the prioritization list
• Follow procedure for documenting contacts
• SOAR Trained
• Other duties as assigned
Knowledge and Experience Requirements
• Bachelor’s degree in Human Services, Counseling, Social Work or related field and/or 4 to 6 years equivalent professional experience required.
• Experience working with socially/economically disadvantaged persons preferred
• Experience assisting with job placement and development activities preferred
• Knowledge of Microsoft Office and data entry.
